Open any Appointment with a Services and press "More" button (Three points on the top right) / Create an Invoice

Also you can set up an Invoice template in Web Cabinet. In order to do it open an Appointment / More / Create Invoice / Configure. In Web Cabinet you will see a list of templates. Download the template and edit.
